Adults and children applying to the NDIS, preparing for plan reviews, or needing clearer evidence about day-to-day functioning

People with complex or interacting conditions (physical, psychosocial, neurodevelopmental, or chronic illness)

Families and carers who need a practical plan that supports safe, reliable daily routines and community participation

We support private clients, and many clients use NDIS self-managed or plan-managed funding (where applicable). Fees can be billed in line with NDIS Pricing Arrangements and Price Limits, which the NDIA updates regularly. NDIS
What's included?
Your FCA package typically includes:

Comprehensive clinical interview via telehealth

Review of your key documents (such as GP letters, specialist/therapy reports, medication lists, school or work information where relevant)

Standardised measures such as, but not limited to, WHODAS 2.0 and COPM

Clear mapping to NDIS criteria (s.24 “substantially reduced capacity” and s.34 “reasonable and necessary”)

Practical recommendations for Core supports, Capacity Building therapies, and Assistive Technology

Outcome tracking plan so progress can be shown over time
