1. Book Your Assessment

You can make a booking via our website by filling the booking form below, and our team will be in touch with you shortly.

2. Telehealth Assessment

Once the Service Agreement is signed, our staff will contact you to schedule the appointment. The occupational therapist will send the telehealth link one day prior and provide simple step by step instructions to join.

3. Report Writing

After the appointment, the occupational therapist will write the report using the interview and any supporting documents. Time frame is around two weeks.

4. Receive Your  Report

Once the report is completed, the occupational therapist will send you a draft to review. Any changes will be made based on your feedback, then the report will be finalised and provided as a PDF.

Home Living Assessment
 

A Home Living Assessment is a specialized report that evaluates a participant’s housing needs to determine the most suitable living arrangement, such as Supported Independent Living (SIL) or Specialist Disability Accommodation (SDA).

It provides the NDIS with crucial evidence regarding the level of 24/7 support, home modifications, or specific design features required to ensure the participant can live safely and independently.
